Question: Why would metabolically healthy person have
low HDL?
-typically HDL is opposite of TG
-when TG rises, body processes these fats with some
enzymes and metabolizes HDL…
therefore HDL does down
We don't fully understand why some
people have low HDL
We treat with various drugs,
but nothing has worked

In this current study we focused only on those with
high HDL, being a sign of good metabolic health…
-in their cases: HDL up; TG down; and LDL also UP
Definition of low HDL: below 40
-some were born with levels below 40
-they usually have a chronic disease

Question: why not study for longer?
-that would be great, but remember, these are extreme
differences: LDL of 272 is very high [normal is 130]
-5 years is long enough exposure to high LDL
for atherosclerosis to start to form

TG are highly variable...tend to go up and down related
to carbs and other dietary factors
-but, over time, low carb eating should tend to make
HDL go up and TG go down

I am a preventive cardiologist
-I have patients like this, with very high LDL, but are
fit, thin, high HDL and low TG
-should I put them on a statin and tell them get off keto?
-the literature for these patients
is very scarce
