This is a gimmick boss fight. The huge dragon is suffering from scarlet rot and is kept alive by eating away at her own children. To defeat her the bond between her and her spawns has to be severed. You have to kill 5 of them in one go, each kill inflicting 17500 points of damage on the mother.
